Patient Center
Everything you need before your first appointment — printable paperwork, what to expect, and insurance information.
Prefer to skip the printer? These forms can be filled out and signed right on your phone, tablet, or computer — complete them before your first visit.
Print and complete the New Patient Forms before your first visit — it usually saves 15–20 minutes at check-in. If you can’t print at home, arrive 20 minutes early and the front desk will have paper copies ready.
Depending on your complaint, Dr. DeFabio may ask you to complete one of these standardized outcome questionnaires. They establish your functional baseline so progress can be measured objectively — bring the one that matches your primary complaint if you’d like a head start.
Medicare and additional condition-specific packets are available from the front desk — call (908) 771-0220 if you’re unsure which forms apply to you.
Plan for 45 to 60 minutes. Your first appointment is devoted to understanding your problem — a structured history, postural analysis, range-of-motion testing, orthopedic stress tests, and neurological screening. Bring your insurance card and any prior imaging or labs so the doctors can review the full picture.
Dr. DeFabio reviews the findings with you in plain language, explains the working diagnosis, and presents a care plan with realistic timelines and measurable goals. Treatment begins only when the diagnosis supports it.
We accept Aetna, Blue Cross Blue Shield, Cigna, and many other plans — call to confirm yours: (908) 771-0220.
Call the office or send a callback request — the Berkeley Heights team will confirm your appointment time.
